Form

Purpose

Honors Application (pdf)

Students wishing to join the Honors College should complete this form and submit it to the Honors office. For submission options, click here.

Honors Graduation Application (pdf)

This application must be submitted to the Honors office (Mail Station #51) by the term's deadline date (see Honors Calendar). Students must have completed 12 credit hours of course work in Honors while maintaining a 3.5 GPA to qualify. Honors graduates will receive an Honors medallion, a notation on the transcript, and a gold Honors seal on the diploma. This form must be signed by the student and the Honors Coordinator.

Honors Graduation Application (doc) This graduation application may be downloaded, completed, and e-mailed to honors@palmbeachstate.edu. Type your name in the signature box. Your accompanying e-mail will serve as your "signature."

Request for Letter of Recommendation (pdf)

Honors students needing a recommendation letter from the Honors Coordinator should print out this form and submit it to Academic Services, Honors Coordinator (Mail Station 51).

Honors Diploma Worksheet (doc) This worksheet can be used to track your progress towards the Honors Diploma. Formatted in Word, the form automatically updates point counts as you accumulate them.

Form

Purpose

Contract (doc)

This form is used by the student who wishes to "honorize" a class by completing an Honors Project. The form must be filled out and signed by the student, instructor, and Associate Dean for that department. This is a Word template. It can be filled out online by tabbing from field to field, or it can be printed and filled out by hand.

Contract (pdf) This form is used by the student who wishes to "honorize" a class by completing an Honors Project. The form must be filled out and signed by the student, instructor, and Associate Dean for that department. This is a fillable Adobe form.

Checklist (pdf)

This checklist describes the Honors Project Contract process. Academic Services attaches the checklist to the contract after it is approved and returns the contract and checklist to the instructor.

Cover Page (pdf)

This is a sample of the cover page that must accompany all Honors Project Contracts regardless of whether the MLA or APA style sheet is used.

Pay Grade / Form (pdf)

This form is for faculty who are mentoring Honors projects. It should be submitted to Academic Services (Mail Station #51) with the completed Honors project(s).

HONORS POINT SYSTEM FORMS

Purpose

Service Activity Form (pdf)

This form is used by the student to record volunteer work for honors points. The form should be completed by the student and activity supervisor and submitted to the Honors office in a timely fashion. Students will receive a confirmation email when forms are processed.

Leadership Activity Form

This form is used by the student to record leadership activity for honors points. The form should be completed by the student and advisor and submitted to the Honors office in a timely fashion. Students will receive a confirmation email when forms are processed.


SCHOLARSHIP FORMS

Purpose

Palm Beach State Honors Scholarship Application (pdf)

To apply for this Honors scholarship, students must have a minimum cumulative 3.6 GPA, have completed 6 credits of Honors coursework in previous terms, be currently enrolled in an Honors course or completing an Honors project, and have no more than 4 withdrawals on their college transcripts. Application does not guarantee an award. NOTE: PALM BEACH STATE HONORS SCHOLARSHIPS ARE NOT AWARDED DURING THE SUMMER TERM.

Honors Book Stipend Application (pdf)

To apply for the book stipend, the student must be a Presidential Scholarship recipient (upon high school graduation)and taking an Honors course work while maintaining a "B" for the course. Applications for reimbursement are submitted to Academic Services (Mail Station #51) after the course has been completed and the grade received. The student must resubmit this application for each semester the student is eligible for the book stipend. NOTE: HONORS BOOK STIPENDS ARE NOT AWARDED DURING THE SUMMER TERM.
